In order to become a Zoom Rides Member Liability Release
- The information provided to the Zoom concierge is accurate and completely true. Any falsehood stated on the application disqualifies your use of the Zoom service.
- To become a Zoomer and use the service, you must provide your name, address, phone number, and email for our records.
- The caregiver will stay with the Senior upon request for an additional fee.
- Zoom has the right to refuse service to customers that are being disruptive, and disrespectful to the caregiver assigned.
- By registering to become a member of Zoom, I attest that I am physically fit to ride in a vehicle without assistance.
- When scheduling a service you will be quoted an estimate of the applicable fees before you confirm your reservation.
- In the event of an accident or sudden illness, I authorize Zoom to arrange for medical care.
- In case of emergency/snow/inclement weather, a caregiver may cancel the service to keep herself & client safe.
- Once a service is scheduled please give 24-hour notice to avoid a cancellation fee.
- I understand that there is no additional commercial insurance.
- I understand that I voluntarily participate in a ride with full knowledge that there is a risk of personal injury or death when being driven to the requested location. I release the driver and caregivers from any liability for personal injury, property damage/loss, or wrongful death caused by an accident, whether due to negligence or otherwise.
- I understand that this release is to be effective during all the terms and at all times that I am registered with Zoom, LLC. If you do not agree with these terms you may not access or use the service.

By using the Zoom service you agree with these terms and do not hold Zoom or any employee of Liability. Zoom will make every effort to help with any disputes.
